The Records Division's main function is the management of all the documentation essential to the operation of this Department.  This includes providing services such as public records requests, license application processing and management of daycare regulations.  In an effort to better serve the public, many of these forms are offered on this website.

All forms on this website are in Adobe Acrobat PDF format.  Please read the instructions for each form carefully.  Completed forms and payment will be accepted at the police station between 8:00 am and 5:00 pm on weekdays (excluding holidays).   If you have any questions, feel free to call or email us.

Pet License Application Dog Cat Spayed/Nuetered Dog Spayed/Nuetered Cat	$10.00 $2.00 $6.00 $1.00	Pet licenses expire annually on May 31ST.  For example, if you were to purchase a pet license today, it would expire May 31, 2014.  Fees are half-price between February 1ST and May 31ST.
Fingerprinting Service Chubbuck Residents Other Residence	$2.00 $5.00	Fee is per card.  Fingerprinting done between 9:00 am and 1:00 pm.  You must provide the encoded print card.
ICCP Care License Initial License License Renewal	$75.00 $25.00	Only applicable for ICCP eligible child care of relative.
Child Care License Initial License License Renewal	$75.00 $25.00	Required for all employees, volunteers and residents twelve or older at same address of child care facility.
Private Investigator License Initial License License Renewal	$75.00 $25.00	Required for business within city limits. Apply in person at Police Department.
