License Renewal Form
- Daycare licenses issued by this agency expire after one year at the end of the month issued. You may fill out this renewal and submit it either in person or by mail.

Daycare Inspection Form
- Daycares licensed by the City of Chubbuck may be inspected at any time without
notice by any employee duly appointed by the Mayor. Failure to pass inspection
will result in either a warning, a misdemeanor citation or suspension of licensing
depending on circumstance. The inspection form is provided so that minimum
standards are maintained.
